Some monitors use HRV at respiratory frequency or respiratory sinus arrhythmia (RSA) as a method of assessing anaesthetic depth. This is useful, but depends on an intact autonomic nervous system and healthy myocardial conducting system. Beta-blockers, conduction abnormalities, autonomic neuropathy and sepsis all cause problems. EEG recordings change from irregular to more regular patterns when anaesthesia deepens. Similarly, FEMG quiets down as the deeper parts of the brain are increasingly saturated with anaesthetics. Entropy measures the irregularity of EEG and FEMG signals.

Evaluation of depth of anaesthesia (DoA) is critical in clinical surgery. The SDI is generated using heart rate variability (HRV) in the time domain and is based on observations of data distribution differences between two consecutive 32 s HRV data segments. A comparison between SDI results and expert assessments of consciousness level revealed that the SDI has strong correlation with anaesthetic depth.

They also stated that HRV would be more suitable to monitor DoA. To my own knowledge EEG based indices like BIS or GE entropy are considered as gold standards and are widelly used worldwide for DoA evaluation. I think authors should provide a more exhaustive literature references list to affirm that EEG is not suitable for DoA monitoring.

General anaesthesia or general anesthesia (see spelling differences) is a medically induced coma with loss of protective reflexes, resulting from the administration of one or more general anaesthetic agents.
